The inspector’s account

On January 16, 2026, Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced visit to the facility for the purpose of conducting the required annual inspection. LPA explained reason for visit and was greeted and granted entry by Director of Operations. During the visit, Director of Operations contacted facility administrator (AD) Michael Villareal about visit. For this visit, there are two staff members on duty, both of which are background cleared and associated. Program Director Andrew Crawford later arrived to assist with the visit.

LPA toured the interior and exterior portions of the facility with Director of Operations and Program Director. The facility is a single level structure and is licensed for six clients aged 18 and over. The facility is a Social Rehabilitation Facility (SRF) licensed for six ambulatory clients. For this visit, there are no clients in care.

There are a total of three client bedrooms, all of which are shared client rooms. There is one office. LPA toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Smoke and carbon monoxide detectors were tested and operational. There are a total of two full bathrooms. Bathrooms were observed to be in good repair, toilets were operational. Water temperature in restrooms were measured to be at 102.2 and 102.7 degrees Fahrenheit.

Sharp items and knives were locked in a kitchen cabinet and inaccessible to clients in care. Fire extinguisher was charged, mounted and located in the kitchen. Fire extinguisher was dated and tagged for the date of June 25, 2025. LPA observed the emergency disaster and evacuation plan, which is posted in the hallway. Continued on LIC809-C.
