Facility Evaluation Report
Licensing Program Analyst (LPA) Jessica Cho conducted an unannounced visit to the facility for the purpose of the annual required inspection. LPA was greeted and granted entry by the front desk staff. Rabbi Marcus was informed by telephone and arrived approximately 9:25am.
Per Rabbi Marcus, facility is currently not operating as of 02/28/2025. LPA observed no clients present.
At approximately 9:30am, LPA toured the facility and observed the following: Facility consists of a large multipurpose room, office space, kitchen and two restrooms. There are no outside areas for clients. LPA reviewed the infection control and emergency disaster plans. Plans are thorough and complete. LPA observed activity supplies. LPA observed no health or safety concerns during visit. Licensee was informed to advise LPA when facility accepts their first client.
Based on the observations made during today's visit, no deficiencies are being cited.
An exit interview was conducted with Rabbi Shneur Zalman Marcus, and a copy of this report was provided at the end of the visit. .
