Facility Evaluation Report
Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced visit to the facility for the purpose of the annual required inspection. LPA arrived and there are no staff present. LPA contacted Rabbi Marcus by telephone who stated would arrive shortly. Rabbi Marcus arrived at approximately 8:10 AM and LPA was granted entry.
Per Rabbi Marcus, facility is currently not operating as 02/28/2025. LPA observed no clients present.
At approximately 8:20 AM, LPA toured the facility and observed the following: Facility consists of a large multipurpose room, office space, kitchen and restrooms. There are no outside areas for clients. LPA reviewed the infection control and emergency disaster plans. Plans are thorough and complete. LPA observed activity supplies. LPA observed no health or safety concerns during visit.
Licensee to forward a copy of Administrator Certificate to LPA by close of business 07/18/2025.
Licensee to advise LPA if facility decides to accept new clients.
Based on the observations made during today's visit, NO deficiencies are being cited. Exit interview conducted and a copy of this report was left at the facility.
