Facility Evaluation Report
Licensing Program Analyst (LPA) Claudia Gutierrez made an announced visit to the facility for purpose of conducting a pre-licensing inspection. LPA met with designated Administrator (AD) Mark Hansen and Consultant Westan Harmer. An application to operate an Adult Day Program (ADP) for (60) capacity, (60) ambulatory, (0) non-ambulatory, and (0) bedridden clients was received by CCL on 1/17/2023.
The facility is a one-story commercial building with one main room, two bathrooms, two staff offices, an activity room, a kitchen, a storage room, an electrical room, and a private leisure room. LPA observed the See Something, Say Something poster (PUB 475) in the entrance way of the facility. There is no backyard or outdoor area.
All and any toxic chemicals, cleaning solutions, laundry toxins and disinfectants are inaccessible to clients and will be stored and locked in one of two staff offices.
Medication will be stored in a cabinet located in a locked staff office. First aid kits are located throughout the facility including one that will be centrally stored with medication. First aid kits have all the required elements.
Records will be kept locked with medication.
