Facility Evaluation Report
Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced visit to The Pleasantview Home. The purpose of today’s visit was to conduct the annual required inspection. LPA was allowed entry into the home and explained the reason for the visit. Facility is licensed for 5 non-ambulatory residents and one bedridden. Facility has an approved hospice waiver for 5 residents and the home currently has 5 residents, with 2 residents on hospice. Administrator Mariza Oliva has an administrator certificate expiring on 05/04/2026.
LPA Lyman along with Caregiver Sosing toured the facility at 10:02 AM. LPA toured the physical plant, checked food service, facility documentation and the first aid kit. The home consists of 5 resident bedrooms, staff room, living room, dining room, and kitchen as well as 4 restrooms. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. At approximately 10:15 AM, LPA observed a bed and drawers/ clothing in an area designated as storage on facility floor plan. Resident bathrooms were checked. LPA observed unsecured Lysol spray in a common restroom. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 105 degrees F and 107.4 degrees F in all restrooms. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Kitchen appliances were operational during today's visit. Fire extinguishers were fully charged. LPA reviewed the infection control and emergency disaster plans and plans are complete and thorough. Facility conducts quarterly emergency drills with the last drill conducted on 09/09/2025. At 10:30 AM, LPA observed unlabeled cleaning supplies in the unlocked laundry room as well as paint in the unlocked garage. Continued on LIC 809-C dated 12/02/2025
