Facility Evaluation Report

Licensing Program Analyst (LPA) Alvaro Ramirez, Jr made an unannounced case management visit in conjunction with Unusual Incident/Injury Report (UIIR) dated February 9, 2024. LPA was greeted and granted entry by Caregiver Michael Lopez. LPA explained the reason for the visit. Administrator (AD) Josephine Teehankee arrived shortly after.

Per UIIR on February 8, 2024, Licensee was notified by a resident's family member of potential financial abuse by former Administrator Assistant.

During today's visit LPA obtained a copy of staff schedule and Resident Roster. LPA also conducted interviews with residents and staff.

A Health and Safety inspection was conducted, and LPA Ramirez observed no Health and Safety concerns during today's visit.

Based on observations and records reviewed during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted with facility representative and a copy of this report was provided at the time of exit.
