Facility Evaluation Report
Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility in order to conduct the required annual inspection. LPA arrived at facility, was greeted and granted entry by Program Director Gaye Satterfield after explaining the purpose of the visit.
LPA accompanied by staff conducted a tour of the program's physical plant. There are twenty-one (21) clients present in the building on the day of the visit, which staff states is approximately the average attendance at the time of the inspection. A musical activity is observed to be in progress during the visit.
LPA reviewed the Disaster and Emergency Plan, Infection Control document, as well as staff records for a total of eight randomly selected staff members along with a sample of six client records maintained at the facility. Records are complete with the exception of one physician report which will be provided by licensee at the earliest convenience. Staff members have been adequately cleared for background and health status and are all verified to be associated to the licensed location in Guardian.
Facility appears to be clean, sanitary and free of odors in all areas inspected. Bathrooms observed are equipped with grab bars. Cleaning supplies and other toxic substances are locked. Water temperature was measured to be 106F in the kitchen and at 112F at the bathroom's faucet.
LPA observed a sufficient supply of food and water for snacks as the facility gets daily meal delivered. Medication is centrally stored under lock for one client only.
