Facility Evaluation Report
On today's date Licensing Program Analyst (LPA) William Vanegas made an unannounced visit for the purposes of completing a required annual inspection. LPA was greeted and granted entry to the facility by facility staff. LPA advised the purpose of the visit, and Assistant Administrator (AAD) Cherry Lyn, was notified via telephone AD joined shortly after to assist with the annual inspection. LPA accompanied by AD conducted a tour of the facility and observed the following.
The facility is a one story home with five private bedrooms, two bathrooms, and an attached two car garage. LPA observed kitchen area to be clean and free of any mildew or debris. LPA observed a two day supply of perishable food, and a seven day supply of non-perishable food on hand. LPA observed there to be a refrigerator, microwave, gas stove, dishwasher, washer and dryer in the facility. All appeared to be in good repair and operational.
LPA observed all client bedrooms to be clean and free any mildew and debris. LPA observed bedrooms to have all required furnishings such as a reading lamp, a chair, a bed, clean linens in good repair; meaning no strains or tares, and enough storage space to store away personal belongings. Bedrooms are large enough to walk about freely and entry ways are not obstructed.
LPA observed resident restrooms to have functional toilets and faucets. LPA observed restrooms to have all required furnishings such as grab bars, slip resistant floor matts, and a shower chair. The hot water temperature tested to be between 96.8 and 105.7 degrees fahrenhight.
