The inspector’s account

On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry by facility administrator Amelia Acevedo.

There is currently only one resident in care who is not receiving hospice care at this time. Administrator stated that the property was listed for sale and operations were winding down pending a new placement for the remaining resident in place. Other residents are stated to have moved out of the facility in July and August 2025. LPA observed the single resident relaxing in the dining area and in their bedroom and interviewed the resident in question. LPA accompanied by facility staff toured the physical plant. The facility is a two-story house with an attached garage, with three bedrooms for use by residents on the ground level. The upper level is strictly for use by the licensee as confirmed during the visit.

Bedrooms appear clean and sanitary. No postural supports are in use at the time of the visit. All resident bedrooms have the required furnishings. Bathroom appears clean and sanitary and is equipped with grab bars and slip mats. Hot water temperature measured at 119F.

LPA observed the kitchen has a minimum two (2) day perishable and seven (7) day non-perishable food supply. Sharp items, cleaning supplies and the medication central storage are verified to be secure. There are two fire extinguishers present which are verified to be charged. Carbon monoxide and smoke detectors were found to be present and operational.
