The inspector’s account

Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA met with Witness 1 who reported the facility is being prepared for a new tenant and is not a facility. LPA verified there are no residents at the facility and the facility is no longer operating. An exit interview was conducted and a copy of the report left with Witness 1. Witness 1 declined to sign the report.
