Facility Evaluation Report

On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility for the purpose of delivering findings into the investigation of complaint 22-AS-20230901120346 filed against facility # 306000639 which was formerly licensed at this location. LPA was greeted and granted entry after introducing himself and stating the purpose of the visit before delivering findings and the corresponding report to administrator Michelle Nesbitt over the phone. Permission was given to caregiving staff to sign on the administrator's behalf.

An exit interview was conducted and a copy of this report was provided to a facility representative.
