The inspector’s account

Licensing Program Analyst (LPA) Jessica Cho made an unannounced Case Management visit for the purpose of of conducting a health and safety inspection. LPA was greeted and granted entry by Care Staff Rodney Monge. Administrator (Admin) Laura Palomino was advised of the visit by telephone and arrived on premise approximately 10am.

LPA observed two staff on duty and observed three clients present. Admin stated that Staff #1 (S1) was terminated on November 11, 2025. LPA conducted a health and safety check on the clients present and confirmed all three clients were doing well. LPA observed the facility to be clean and organized. LPA observed facility has a 2-day supply of perishables and 7-day supply of non-perishable food as required by regulation including emergency food and water. LPA observed the electricity and water running, facility maintained soap and paper towels, medications/sharps/toxins were properly secured and stored. The fire extinguisher was serviced on September 11, 2025, and the dual functioning smoke and carbon monoxide detectors were tested and operational.

LPA obtained additional records during today's visit: Client Roster, Personnel Report, Face Sheet, Admission Agreements, Consent for Medical Treatment, Behavioral Report, Daily Progress Notes, Behavioral Objective for Client #1 (C1) as well as photo identification of S1. LPA requested copy of the hospital discharge summary, and Admin will submit it to LPA by close of business today.

No deficiency cited during today's visit.

An exit interview was conducted with Administrator Laura Palomino, and a copy of this report was provided at exit.
