Facility Evaluation Report
On July 22, 2026, Licensing Program Analyst (LPA) Jessica Cho made an unannounced visit for the purpose of conducting the Required- 1 Year inspection. LPA was greeted and granted entry after explaining the reason of the visit to Direct Support Professional (DSP) Leticia Granda. Administrator (Admin) Laura Palomino arrived on premise approximately 11:10am. LPA observed Admin has a valid certificate expiring January 23, 2027.
This facility is a Level 4G, two story residential group home comprised of four (4) private client bedrooms and two (2) client bathrooms. The client census is three (3). LPA observed one staff and one client present during the visit while the remaining two clients were out in the community. Facility is operating within the conditions and limitations specified on the license. LPA toured the premise accompanied by Admin Palomino. LPA observed all client bedrooms had the required elements with ample lighting. All bathrooms were clean and operational. The hot water temperature measured at 118.0 degrees Fahrenheit in both bathrooms. There were a clean supply of linens. LPA observed sufficient two-day supply of perishables and seven-day supply of non-perishable food. Toxins, disinfectants, sharps, and medications were secured and inaccessible. LPA observed the fire extinguisher was mounted, charged, and serviced on September 11, 2025. The indoor temperature remains at a comfortable range. All smoke/carbon monoxide detectors were tested and operational. There are no bodies of water on premise. Facility maintains emergency food, water, and disaster supplies noted on the Emergency Disaster Plan including the first aid kit. LPA toured the outside grounds. Both exit gates on each side of the property are operational. The outdoor passageway is clear and no tripping hazards observed. There are sufficient seats under a shaded area.
