Facility Evaluation Report

Licensing Program Analyst (LPA) arrived unannounced on a Plan of Correction (POC) visit. LPA was greeted and granted entry into the facility by Care Staff (CS) Marcie Rechlin and Administrator (Admin) Laura Palomino was advised the reason for the visit by telephone.

Regarding Complaint Control #: 22-AS-20260107104724, the facility was issued a Type B deficiency on January 9, 2026:

80087 Building and Grounds - Facility complied with the terms of the POC by removing the stains and residues from both refrigerators.

Regarding Complaint Control #: 22-AS-20260107104724, the facility was issued a Type B deficiency on January 28, 2026:

80072(a)(3) Personal Rights - Facility complied with the terms of the POC by ensuring the garage fridge/freezer remains unlocked and the pantry items such as snacks were made available for client access.

An exit interview was conducted with Administrator Laura Palomino, and a copy of this report including the Letter of Deficiency Citations Cleared were provided at exit.
