Facility Evaluation Report

Licensing Program Analyst (LPA) Jessica Cho made an unannounced visit for the purpose of conducting a Case Management- Deficiencies due to an observed deficiency in connection to Complaint Control No: 22-AS-20250425145035 . LPA met with Administrator Laura Palomino and explained the reason for the visit.

During the course of the investigation, it was observed during the audit that the medication label, Quetiapine Fumurate 50 mg, for Client #1 (C1) was altered from a PRN and relabeled as AM, Noon, and Bedtime.

Therefore, based on observations, interviews, and record review, a deficiency is being cited as per the Title 22, Division 6, Chapter 1 of the California Code of Regulations. Refer to the LIC809-D.

An exit interview was conducted with Administrator Laura Palomino, and a copy of this report including the LIC811 and the appeal rights were provided at exit.
