Facility Evaluation Report
Licensing Program Analyst (LPA) Jessica Cho arrived at the facility unannounced for the purpose of conducting the Required 1-Year annual inspection. LPA was greeted and granted entry by Caregiver Tamara Barrera after stating the reason for the visit. Administrator (Admin) Pilar Layton arrived approximately 9:30am. Admin has a valid certificate expiring December 23, 2026 per the Active Administrator Certificate Holders list.
The following was observed during the inspection:
This is a two story property located in a residential neighborhood. Facility operates within the conditions and limitations specified on the license. LPA observed six residents in care of which none are receiving hospice services. LPA observed two caregivers on duty and verified fingerprint clearance and association statuses. There are two private resident bedrooms, two shared resident bedrooms, two resident bathrooms, and one private staff bedroom with a separate entry way from the patio. The staff bedroom has a private bathroom and a kitchenette. Across the staff bedroom in the backyard, a new additional dwelling unit (ADU) was constructed within the past year. At the time of the inspection, the required fire clearance had not been completed or obtained. All common areas were inspected including the office which doubles as a medication room, the attached two car garage, and the second floor occupied by the licensee's family member consisting of one bedroom, bathroom, living area, and kitchen. LPA toured the first floor and observed the residents' bedrooms were appropriately furnished. Beds and bedding supplies were in good condition, adequate lighting was provided, sufficient storage space for each residents' personal belongings were observed. Bathrooms were found to be in compliance, clean, and operational. The hot water temperature measured at 117.3 and 113.7 degrees Fahrenheit in the resident bathrooms. Toxins, disinfectants, sharps, and medications were secured and inaccessible.
