The inspector’s account

Licensing Program Analyst (LPA) Jessica Cho made an unannounced Plan of Correction (POC) visit to follow up on the deficiencies issued during the annual inspection conducted on September 17, 2025 from 1:30pm-5:00pm. LPA was greeted and granted entry Caregiver Sixto Pasqua. LPA spoke to Administrator Sherry Dizon by telephone and explained the purpose of the visit and authorized staff to sign the report on their behalf.

Regarding, 1569.695(c), Health and Safety code indicates that "a facility shall conduct a drill at least quarterly for each shift... taking into account different emergency scenarios." Emergency drill was conducted on September 26, 2025 per today's review of the disaster log.

Regarding, 87303(a), regulation indicates facility is "clean, safe, sanitary, and in good repair at all times." LPA observed a presence of mold in the grout and a small hole above the shower in one out of the six resident bathrooms inspected during the annual inspection. On today's date, LPA observed the hole was patched and the shower and grout appeared clean and free of mold for all bathrooms.

Facility has complied with the terms of the POC.

An exit interview was conducted with Caregiver Jordan Frias, and a copy of this report including the Letter of Deficiency Citations Cleared were provided at the end of the visit.
