Facility Evaluation Report
Licensing Program Analyst (LPA) Jessica Cho arrived at the facility unannounced for the purpose of conducting the Required 1-Year annual evaluation using the CARE Inspection Tool. LPA was greeted and granted entry by Care Staff Lorna Cole and stated the reason for the visit. Administrators (Admins) Candace Bates followed by Randall Bates arrived on premise to assist with the inspection. The administrator's certificates for Randall Bates and Candace Bates are valid expiring on March 16, 2026 and September 7, 2026.
The following was observed during the inspection:
This is a two story, Level 2 home located in a residential neighborhood comprised of three shared client bedrooms and two client bathrooms. There is an additional bedroom and private bathroom for the staff. Facility operates within the conditions and limitations specified on the license. LPA observed three clients getting ready to leave for the day program. LPA observed one care staff on duty and verified fingerprint clearance and association statuses including the administrators. All common areas were inspected which includes the laundry room and the attached two car garage. LPA toured both floors of the facility. Client/staff bedrooms are on the second floor. The evacuation chair is present by the stairs. LPA observed and inspected all bedrooms. The client bedrooms' were appropriately furnished, beds and bedding supplies were in good condition, adequate lighting was provided, and sufficient storage space for each clients' personal belongings were observed. All bathrooms were found be in compliance, clean, and operational. The hot water temperature in both client bathrooms measured at 113.7 and 113.3 degrees Fahrenheit. Toxins, disinfectants, sharps, and medications were secured and inaccessible. The fireplace behind the sofa is screened. LPA observed ample two-day supply of perishables and seven-day supply of non-perishable food.
