Facility Evaluation Report
On September 24, 2025, Licensing Program Analyst (LPA) Jenifer Tirre conducted an unannounced required visit. LPA was greeted by staff and granted entry after stating the purpose of the visit. Licensed Vocational Nurse (LVN) Johanna Millan was available to assist with the facility inspection. Clinical Director Anthony Resendiz arrived shortly after.
The facility is licensed for six (6) ambulatory clients. Facility is a short term residential crisis program home. Currently, there are six (6) clients present during today’s visit.
Facility is a two story residential home with attached garage. The facility has three (3) shared client bedrooms, two (2) staff offices, three (3) bathrooms, kitchen, activities room and living room.
LPA conducted a tour of the physical plant accompanied by LVN Millan, and the following was observed: Facility has a swimming pool in backyard with secured gate on premises. Rooms were inspected. Beds and bedding supplies were in operational condition, lighting was provided, and storage for the client's personal belongings was observed. Bed linens and bath towels were available during the visit. Bathrooms were operational with water temperature measured at 120.0 degrees F. A comfortable temperature of 76 degrees F. was maintained in the facility.
LPA observed the facility to be furnished at the time of the visit. Storage areas for sharps objects and cleaning supplies were stored and not accessible to clients. The kitchen was inspected, and facility had sufficient two day perishable and seven day non-perishable food.
