Facility Evaluation Report

Licensing Program Analyst (LPA) Jessica Cho made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry by Administrator Erwin Romwel Arceo and notified Licensee Estrella Quinto via telephone at 9:01am explaining the reason for the visit.

On today's inspection, LPA observed five residents in care with two staff/administrators on duty. LPA was advised by Licensee Quinto that this facility went through a Change of Ownership (CHOW) and is now currently operating under licensed facility, RCO Home Care LLC #306006695 as of August 19, 2025. Licensee Quinto acknowledged to permanently close the facility and surrendered the original facility license during the visit. LPA received an email on today's date confirming request to close facility. The Department will proceed with the closure with an effective date as of August 19, 2025.

An exit interview was conducted with Licensee Erwin Arceo authorized by Licensee Quinto, and a copy of this report was provided to Licensee Quinto via email.
