Facility Evaluation Report
Licensing Program Analyst (LPA) Jessica Cho arrived unannounced to follow up on the deficiencies and Technical Violations (TVs) issued during the annual inspection conducted on April 9, 2025, 8am-12:20pm. LPA was greeted and granted entry by Caregiver Mary Gee Dingcong and was provided an explanation for the visit.
Regarding, 87555(b)(27) , General Food Service Requirements regulation indicates that "all kitchen areas shall be kept clean and free of insects. " All fruit flies were eradicated per inspection.
Regarding, 87411(f), Personnel Requirements regulation indicates that "all personnel... shall be verified by a health screening, including a chest x-ray or an intradermal test, performed by a physician not more than six (6) months prior to or seven (7) days after employment or licensure." The TB test for the staff was submitted in a timely manner.
Additionally, LPA reviewed the Technical Violations (TVs). LPA reviewed the amended Emergency Disaster Plan (LIC610 E) and observed the kitchen refrigerator/panty were clean, and the exit gate self-latching.
Facility has complied with the terms of the Plan of Corrections (POCs).
An exit interview was conducted with Caregiver Mary Gee Dingcong authorized by Administrator Josie Teehankee, and a copy of this report including the Letter of Deficiency Citations Cleared were provided at the end of the visit.
