The inspector’s account

Licensing Program Analyst (LPA) Jessica Cho arrived unannounced for the purpose of conducting the Required 1-Year annual evaluation using the Care Inspection Tool. LPA was greeted and granted entry by Caregiver Mary Gee Dingcong and explained the reason for the visit. Administrator (Admin) Lance Bryant II arrived on premise at or approximately 9am to assist with the inspection. The administrator's certificate for Josephine Teehankee expires on November 19, 2026 and on January 27, 2027 for Rafael Teehankee.

The facility is a two story structure and is licensed to operate for six non-ambulatory residents of which one may be bedridden and maintains a hospice approved for six. During today's visit, LPA observed six residents in care and two staff on duty. LPA toured the physical plant with Admin and House Manager Ara Malaika David and observed the following: There are six resident bedrooms and three resident bathrooms on the first floor and three staff bedrooms and two staff bathrooms on the second floor. Only staff are allowed access to the second floor. Resident bedrooms had all required furnishings. Bathrooms were found to be in compliance, clean, and operational, and the hot water temperature in the resident bathrooms measured at 108.6, 107.4, and 106.4 degrees Fahrenheit. All common areas were inspected including the attached two car garage which doubled as a laundry area and to store extra food and disaster supplies. LPA observed at least 72 hour supply of emergency food and water. Toxins, disinfectants, sharps, and medications were secured and inaccessible. LPA observed sufficient two-day supply of perishables and seven-day supply of non-perishable food available in the kitchen and garage. LPA observed fruit flies in the facility mainly in the kitchen area. LPA toured the exterior portion of the facility. The outdoor passageway is free of obstruction. The exit gates were self-closing however was not self-latching for one gate. LPA observed sufficient seating and shading. The fire extinguisher was purchased on April 3, 2025. The auditory devices and dual-functioning smoke/carbon monoxide detectors were tested and operational on both floors.
