Facility Evaluation Report

On this day Licensing Program Analyst (LPA) William Vanegas made an unannounced visit for the purposes of conducting a plan of correction visit. Upon arrival LPA Vanegas was greeted and granted entry to the facility by Care Give Rosmie Morota. LPA Vanegas explained the purpose of the visit and began to enquire of the citations issued on July 29, 2025.

LPA Vanegas was not presented with proof of correction; meaning staff on duty did not present proof of completed training being completed in regards to emergency services being contacted upon an emergency or a fall taking place at the facility, additionally LPA Vanegas was not provided with proof of correction in regards to a large sum of funds being refunded to a previous resident in care.

Based on the lack of proof of correction for the citations issued on July 29, 2025 civil penalties were issued on today's date, an exit interview was conducted and a copy of this report was provided to the facility.
