Facility Evaluation Report
On February 6, 2026, Licensing Program Analyst (LPA) Jessica Cho made an unannounced visit for the purpose of conducting the Required- 1 Year evaluation using the Care Inspection Tool. LPA was greeted and granted entry after explaining the reason for the visit to Lead Direct Support Professional (DSP) Michelle Torres.
This facility is a two story residential home comprised of four private client bedrooms and three client bathrooms. The client census is three (3). There are no clients and one (1) staff present during the visit. Facility is operating within the conditions and limitations specified on the license. LPA toured the premises accompanied by DSP Torres. LPA observed all client bedrooms which had all required elements with ample lighting. All bathrooms were clean and operational. The hot water temperature measured at 112.3, 116.2, and 113.2 degrees Fahrenheit. There were a clean supply of linens in the closet. LPA observed ample two-day supply of perishables and seven day supply of non-perishable food. Toxins, disinfectants, sharps, and medications were secured and inaccessible. The facility is equipped with sufficient hand hygiene, cleaning, and disinfectant supplies. LPA observed the fire extinguisher on both floors were mounted, charged, and serviced on January 21, 2026. The indoor temperature remains at a comfortable range. The dual functioning smoke/carbon monoxide detectors were tested and operational. The fire place was screened, and there is no body of water on premise.
LPA toured the outside grounds. The outdoor passageways are clear and free of obstructions. The two exit gates were self-closing and self-latching.
