Facility Evaluation Report
Licensing Program Analyst (LPA) Kimberly Lyman made an announced visit to conduct a follow up pre-licensing inspection. LPA identified herself and discussed the purpose of the visit with Area Manager Tony Duarte. An initial application to operate an Adult Residential Facility was received by CCL on 03/18/2022 for a capacity of four ambulatory clients. Administrator Jennifer Hrabal arrived during the visit.
At 9:09 AM, LPA toured the facility and observed the following:
Burner on the cook-top has been repaired and is operational.
Facility has posted the activity schedule as well as the facility menu. Both items are complete.
Facility has hand washing signs posted in all facility restrooms.
Facility has covid precaution signage at entrance as well as a screening station.
Facility has complied with all corrections and is ready to be licensed.
Exit interview conducted and a copy of this report was left at the facility.
