Facility Evaluation Report
On 07/14/2025 at 12:45 pm, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced annual required visit using the CARE Inspection Tool. At LPA met with the Rodrigo Mateo, Administrator and the purpose of today’s visit was explained. The facility is licensed to operate for (3) of which (1) ambulatory and (2) non-ambulatory developmentally disabled adults with restricted health conditions. Currently, the home has (3) clients. The clients are Harbor Regional Center clients. None of the client have Restricted Health Care Condition nor are not utilizing postural supports and protective devices.
The facility has a current administrator certificate (# 7035331735) for Rodrigo Mateo valid from 05/10/2024 - 05/09/2026 . The facility fees are current. The facility has liability insurance with American Alternative Insurance Corp (NAIC# 19720) with each occurrence at $1,000,000 and general aggregate at $3,000,000 (policy #G8A2CP000015700) effective date 07/22/2024 - 07/22/2025. The facility fees are current.
The facility is a single story home located in a residential neighborhood. The property consists of the following: four (4) bedrooms, two (2) bathrooms, a kitchen, a dining room with a den, a laundry room, a office area, an attached garage with an additional supplies and an outdoor shaded area in the backyard.
