Facility Evaluation Report
On 03/09/2026 at 12:30 pm, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with the Lodervinejoy "Joy" Alipio, Administrator and the purpose of today’s visit was explained. The facility is licensed to operate for (3) non-ambulatory developmentally disabled adults with restricted health conditions The facility has an approved Hospice Waiver for (1) clients. Currently, the home has (2) clients. The clients are Harbor Regional Center clients. The (2) clients have Restricted Health Care Conditions and two (2) of the clients are utilizing postural supports or protective devices. The facility has an annual fee with the balance of $0. The facility has liability insurance for $1,000,000 each occurrence /$3,000,000 general aggregate effective 07/01/2025 - 07/01/2026 with James River Insurance (policy #00093259-6).
The facility is a one story home located in a residential neighborhood. The property consists of the following: 3 client bedrooms, 2 common bathrooms, staff office, living room, kitchen, dining room, attached garage which houses a additional refrigerator, a laundry room with a washer and dryer and an outdoor shaded area with a garden.
Between the hours of 1:35pm - 2:25pm, LPA conducted a records review of (2) client records, (4) staff records, (2) clients Personal & Incidental Records and reviewed the facility disaster plan. All client & Staff records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA reviewed (2) Client Medication Administration Records and did not observed any discrepancies at the time of visit. The last date of emergency drill being conducted is on 03/05/2026.
