Facility Evaluation Report
On 10/07/2025 at 12:10 pm, Licensing Program Analyst (LPA) Zina Brown conducted a Case Management visit to the facility above. For the purpose of following up on a confirmation of removal notification. LPA met withf Jolanda Cartridge(Administrator) and explained the reason for the visit.
A notification letter was generated to notify the licensee of an individual Esohe Ugbogbo confirmation of removal. LPA spoke with China Hampton who explained that Esohe Ugbogbo was an employee who formerly worked at the facility. Esohe Ugbogbo has not worked at the facility since March 1, 2024
During this visit, LPA obtained a copy of the staff schedule (Dated 09/15/2025) from Jolanda Cartridge Administrator, which confirm that Esohe Ugbogbo is not working at the facility. The Department received information an individual Esohe Ugbogbo should not be on site, and this has been verified during today’s visit.
