The inspector’s account

On 10/22/2025 at 1:15pm, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced Case Management - Incident visit at this facility. LPA met with Damieka Lasley (Administrator) and explained the purpose of the visit to conduct a health and safety check to follow up on a incident that occurred on September 28th, 2025.

The Department interviewed Clients (C1-C4 with C3 refusing to be interviewed and walked away) in regards to the incident involving with (C1) & (S1) . An incident occurred on 09/28/2025 at the facility with staff giving client a tattoo on the facility premises inside the garage. The department received the incident report on Thursday, 10/02/2025.

D ue to insufficient time LPA has decided additional time is needed to complete the investigation.

Exit interview conducted with Damieka Lasley (Administrator) and a copy of this report was provided.
