Facility Evaluation Report

On 10/07/2025 at 08:10 am, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced annual case management visit. At 09:10 am LPA met with Celeste Orellana, Program Manager and the purpose of today’s visit was explained.

An incident occurred on 09/28/2025 at the facility with staff giving client a tattoo on the facility premises inside the garage. The department received the incident report on Thursday, 10/02/2025.

During the case management visit, LPA obtained copies of the following documentation:

LIC 503: Health Screening Report - Facility Personnel - dated 8/23/2023

By the end of today on Tuesday, 10/07/2025 the facility will email the following documentation

LIC 601 Identification and Emergency Information - dated 01/23/2025

LIC 625 Appraisal/Needs & Service Plan - dated 10/07/2025

Individual Person-Centered Plan (IPP) - dated 09/12/2024

Due to insufficient time LPA has decided additional time is needed to complete the investigation.
