Facility Evaluation Report

On 10/07/2025 at 08:10 am, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced annual required visit using the CARE Inspection Tool. At 09:10 am. LPA met with Celeste Orellana, Program Manager and the purpose of today’s visit was explained. The facility is licensed to operate for (4) ambulatory developmentally disabled adults ages 18 through 59. Currently, the home has (4) clients. The clients are Harbor Regional Center clients. None of the clients have Restricted Health Care Conditions and is utilizing a protective devices.

The facility has a current administrator certificate (# 7009439735) effective date 08/11/2024 - 08/10/2026 for Celeste Orellana valid from 08/11/2024 - 08/10/2026 . The facility fees are current. The facility has liability insurance with Nationwide Insurance Company of America (NAIC#25453) with each occurrence at $1,000,000 and general aggregate at $3,000,000 (policy #ACPG023059558980) effective date 01/01/2025 - 01/01/2026.

The facility is a one story home located in a residential neighborhood. The property consists of the following: 4 client bedrooms, 3 bathrooms, staff office, living room, kitchen (water temperature tested at 112.6°F), dining room, a washer, attached garage which houses dryer and refrigerator and an outdoor shaded area.
