Facility Evaluation Report

Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. made an unannounced visit in conjunction with complaint 22-AS-20230503123146. LPA Ramirez was allowed entry into the facility and met with Administrator (AD) Juan Garcia Trujillo. LPA explained the purpose of the visit.

On today's visit LPA Ramirez toured the facility and interviewed the AD. During today's visit the AD reported that on December 14, 2023, the facility issued a refund to Resident 1 (R1). During today's visit AD was unable to show how much rent R1 paid since the bank account has been closed. AD agreed to request a copy of the April 2023 bank statement and will provide a copy to LPA by close of business on April 8,2026. AD also agreed to email the communication records between him and R1's daughter.

Due to insufficient information available at this time, LPA to return at a later date.

An exit interview was conducted with AD Garcia and a copy of this report was provided at the time of exit.
