The inspector’s account

On March 13, 2026, Licensing Program Analyst (LPA) Garlli Tat made an unannounced Plan of Correction visit. LPA is following up on a deficiency cited on a case management visit on January 8, 2026. LPA was greeted and granted entry into the facility by staff after explaining the purpose for the visit. Administrator (AD) Eleazar Cuyson was notified via telephone and provided verbal approval that lead caregiver can sign the report on his behalf.

On today's visit, LPA observed three residents in care and two staff present. LPA observed one resident sleeping in the living room and two residents watching television in their rooms. LPA observed residents to be in clean clothes.

LPA observed all internet connected television sets in the facility were working. LPA requested to view caregiver's mobile phone device to ensure it is connected to the facility's Wi-Fi. During the POC visit, LPA interviewed three staff and two residents.

Based on the observations made during today's visit, no deficiencies are being cited per the Title 22 of the California Code of Regulations. The previous citation issued during the case management visit was cleared at time of visit. An exit interview was conducted with an authorized facility representative and a copy of the report was provided.
