Facility Evaluation Report
Licensing Program Analyst (LPA) Celine Rodriguez conducted an unannounced case management to conduct a health and safety check for an incident that was reported to the Regional Office. LPA explained reason for visit and was greeted by staff on duty (S1) Roberto Yee. For this visit, there are a total of 3 clients in care.
On 11/3/25, the department received a death report for client 1 (C1). LPA conducted a file review and interviews and verified that C1's death was due to a pre-existing condition (neuroendocrine cancer) that ended up spreading to C1's liver.
On 10/28/25, S1 observed C1 to be unconscious, to which S1 contacted 911 immediately. Facility contacted C1's responsible party, and 911. C1 was brought to the hospital, to which the hospital confirmed C1's death.
For this visit, no health and safety concerns were observed.
For today's visit no deficiencies were issued per Title 22 Division 6 of the California Code of Regulations. No citations were issued.
An exit interview was conducted with S1 Yee. A copy of this report was provided and explained.
