Facility Evaluation Report

On June 22, 2026, Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced visit to the facility for the purpose of conducting the required annual inspection. LPA explained reason for the visit and was greeted and granted entry by Administrator (AD) Emmy Lou Cinco. For this visit, there is one staff member on duty, who is both background cleared and associated.

LPA observed the Administrator's Certificate for Emmy Lou Cinco, which expires on June 7, 2026.

LPA toured the interior and exterior portions of the facility with AD. The facility is a two level structure and is licensed for six clients from the ages of 18 to 59. The facility is an Adult Residential Facility (ARF) licensed for six ambulatory clients. For this visit, there are a total of four clients in care.

There are a total of four bedrooms, three client bedrooms, all of which are shared client rooms, and one bedroom for staff. LPA toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Smoke and carbon monoxide detector and auditory exit alarms were tested and operational. There are a total of two bathrooms, one is for staff and one is for clients. Bathrooms were observed to be in good repair, toilets were operational, and grab bars and non-skid floor mats were provided. Water temperature in bathrooms were measured to be at 113.7 to 114.2 degrees Fahrenheit.

Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked in a kitchen cabinet and inaccessible to clients in care. Fire extinguisher was charged, mounted and located in the kitchen. Fire extinguishers were dated and tagged for the date of April 9, 2026. Continued on LIC 809-C.
