Facility Evaluation Report
Licensing Program Analyst (LPA) Celine Rodriguez conducted an unannounced visit to the facility for the purpose of conducting the required annual inspection. LPA Rodriguez was greeted and granted entry by facility administrator (AD) Emmylou Cinco and stated the purpose of today's visit.
LPA Rodriguez observed administrator certificate expires on June 7, 2026. The PUB475 "See Something, Say Something" poster was observed to be posted at the entrance.
The facility is an Adult Residential Facility licensed for six ambulatory clients. There are currently four clients in care. For this visit, there were no clients present due to attending day program.
The facility is a two story home. On the first floor, there are a total of three bedrooms, of which two are shared client rooms, and one room is designated for staff. There is one bathroom shared among clients located on the first floor. On the second level, there is one bedroom, one bathroom, and one staff office, of which during this visit, it was verified that clients do not reside on the upper level. LPA Rodriguez toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Restrooms were observed to be in good repair, and toilets were operational. Smoke and carbon monoxide detector and auditory exit alarms were tested and operational.
Hot water was measured at 110.4 degrees Fahrenheit. LPA Rodriguez observed that the facility had a two-day supply of perishables and a seven-day supply of non-perishable food. LPA Rodriguez observed hallways were free of obstructions. There are no bodies of water on the premises. Facility has is a covered patio, and the backyard has two gates, of which both are self-closing and self-latching
