Facility Evaluation Report

Licensing Program Analyst (LPA) Ruth Martinez conducted an case management visit for the purpose to verify facility closure. LPA arrived at facility was greeted and granted entry by Lori Cook, Administrator.

The Department was informed on March 10, 2026, that facility had officially closed on March 15, 2026, and will have no clients in care. On today’s visit the licensee no longer was operating as a licensed facility. LPA was informed that the last client moved out on March 9, 2026, and relocated to another facility. The reason for today’s inspection is to confirm the closure of the licensed facility.

LPA accompanied by Administrator toured the facility and observed no clients in care. LPA observed the home to be empty and found no evidence the home is operating as a licensed facility. Based on the observations, the facility is no longer operating as a licensed facility and is closed.

LPA collected the facility license and a corrected closure letter reflecting the correct facility information.

This report was reviewed with Administrator and a copy of this report was left at facility.
