The inspector’s account

Licensing Program Analyst made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with the Administrator Albert Dahbour. The Administrator's Certificate expires on May 17, 2026. LPA explained the reason for the visit. LPA observed the See Something, Say Something sign (PUB 475) mounted in the entry way of the facility is 20 by 26 inches. LPA and Administrator toured the facility. Facility is licensed for 6 non-ambulatory residents with a hospice waiver for 6. The facility is a one story house with six resident bedrooms, four bathrooms, living room, kitchen, dining room, laundry room, LPA observed that the 3 car garage has been altered, it is a 2 car garage and a room. The area that was for the third car has been converted into a room with a table and chairs. The facility sketch mounted in the entry way of the facility shows the garage but not the converted room. The rest of the garage is used for storage. LPA observed a 3 day supply of emergency food and water. Smoke detectors and the carbon dioxide detector tested operational. The last emergency drill was conducted on April 1, 2025. The fire extinguisher mounted in the dining room is fully charged. LPA observed the kitchen is clean and organized. LPA observed a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. LPA observed knives are kept locked under the kitchen sink. LPA observed cleaning supplies and chemicals are kept locked in a cabinet in the laundry room. Medications are kept locked in a cabinet in the dining room. LPA observed all resident rooms are clean and organized. LPA observed all resident rooms had the required furnishings. LPA observed all bathrooms were clean and operational. Hot water measured 113.1 degrees Fahrenheit. LPA observed a small gap in the floor tiles in the living room.
