Facility Evaluation Report
Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct a proof of correction (POC) visit. LPA met with Administrator Hannah Pichon and explained the reason for the visit. LPA observed the smoke/carbon monoxide detector in the hallway between bedroom 5 and bedroom 6 has been replaced. Smoke/carbon monoxide detectors were tested and are operational. LPA observed all resident medications are stored in their original containers. The Administrator provided documentation that an emergency disaster drill was conducted on July 22, 2025. LPA observed the facility now has a 3 day supply of emergency food and water. LPA reviewed Staff 2's file. Staff 2 now has 20 hours of annual training. All deficiencies have been cleared. LPA provided Administrator with proof of correction letters for each deficiency. An exit interview was conducted and a copy of the report provided.
