Facility Evaluation Report

Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct a proof of correction visit for a citation issued on April 29, 2026 as a result of the complaint investigation for complaint 22-AS-20260428130939 . LPA met with staff and explained the reason for the visit. LPA and staff toured the facility. LPA observed the facility is clean and organized. LPA observed all trash/recycle cans have been cleaned. No odors were detected inside or outside of the facility. No obstacles or hazards observed inside or outside of the facility. The deficiency has been cleared. LPA provided staff with the Letter of Deficiency Citations Cleared. An exit interview was conducted with staff and a copy of the report provided.
