Facility Evaluation Report
108.8. Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by Administrator Voltaire Almacen. LPA explained the reason for the visit. Voltaire Almacen Administrator's certificate expires on September 2, 2027. The facility is a two story house with 7 bedrooms, 4 bedrooms are for clients, 3 bathrooms, living room, dining room, laundry room, kitchen and an attached 2 car garage. Facility is licensed for 4 ambulatory clients. LPA and Administrator toured the facility. LPA observed the fire extinguisher in the hallway is fully charged. The laundry room and garage are kept locked and inaccessible to clients. LPA observed a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. Emergency food and water stored in the kitchen closet. LPA observed the kitchen is clean and organized. All appliances, refrigerator, microwave, 5 burner gas stove and dishwasher are all operational. The 5 burner gas stove lights unassisted. LPA observed all sharp objects are kept locked in a kitchen drawer. Cleaning supplies are kept locked under the kitchen sink. The second floor has 2 bedrooms and one bathroom. Clients are not allowed on the second floor. LPA observed all client rooms had the required furnishings and linens. LPA observed the emergency evacuation chair mounted at the bottom of the stairway. Hot water measured 108.8 degrees Fahrenheit in the downstairs bathroom. LPA observed clean linens in the hall closet. LPA and Administrator toured the backyard. There is a covered patio with a table and chairs to sit outside. LPA observed the exit gates on each side of the house are operational. No bodies of water observed. LPA observed 2 storage sheds in the backyard. Both storage sheds stored old clothes and furniture. Both sheds are kept locked and inaccessible to clients. No obstacles or hazards observed in the backyard. LPA reviewed 1 client record. No discrepancies observed. Client is new to the facility and medications have just been prescribed and not received by the facility yet.
