Facility Evaluation Report
Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Administrator Peter Schenkelberg and explained the reason for the visit. Peter Schenkelberg's Administrator's Certificate expires October 13, 2027. LPA and the Administrator toured the facility. LPA observed the fireplace in the living room is screened. There is a large screen TV, books and magazines and 5 reclining chairs in the living room. LPA observed the See Something Say Something poster (PUB 475) posted next to the front door. Facility is licensed for 6 non-ambulatory residents with a hospice waiver for 6. The facility is a two story house with 8 bedrooms and 6 bathrooms, a living room, dining room, kitchen and a 2 car garage. The second floor is off limits to residents and there is no way for residents to access the second floor. The garage is used for storage and kept locked. LPA observed all resident rooms are clean and organized. All resident bedrooms have the required furnishings. LPA observed all resident bathrooms are clean and operational. Hot water measured 113.0 to 113.3 degrees Fahrenheit. LPA toured the kitchen. The fire extinguisher in the kitchen is fully charged. LPA observed the kitchen is clean and organized. The 4 burner stove lights unassisted and has safety devices so the stove cannot be operated by residents. LPA observed a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. There is a 3 day emergency food and water supply in the kitchen. LPA observed sharp objects and knives are kept locked in a kitchen cabinet. LPA observed medication is kept locked in a kitchen cabinet. Cleaning supplies are kept locked under the kitchen sink. LPA inspected the first aid kit. The first aid kit has all the required elements. The last emergency drill was conducted on March 1, 2026. Smoke detectors and the carbon monoxide detector tested operational. LPA observed the garage is kept locked and used for storage. LPA and the Administrator toured the backyard. No bodies of water observed.
