Facility Evaluation Report

Licensing Program Analyst (LPA) Ruth Martinez arrived at facility and met with Brittany Stipes, Program Director and Adam Rodriguez, Program Manager for the purpose of conducting a Prelicensing inspection. An initial application to operate an Adult Day Program for 50 ambulatory clients was submitted to CCL on August 20, 2024.

Structure . The facility is a one story building located in a business office complex. It consists of a long open floor plan with rooms designed for multiple use areas for activities with clients. There is a reception area in the main entry, client rooms are as follows: gym, functional skills, art therapy, vocational skills, sensory room, music therapy, recreation room, computer lab, facility has four office spaces for staff, a conference room, two restrooms and there is a kitchen equipped with a refrigerator, sink, and microwave oven. There are storage cubies along the walls in the hallways located in clients’ rooms for clients use. Facility will operate Monday through Friday from 7:30 a.m. through 3:30 p.m. with the program’s operation hours of 8:30a.m. to 2:30p.m.

Posted on the reception area and throughout the facility. Appliances . Refrigerator and a microwave. Food Service . Clients will bring own food & snacks. Facility will provide snacks as well. Drinking Water. Available & located inside the building for clients use. Water Temperature . Tested at 111.5 Fahrenheit degrees. Toxins . Locked in the janitor room located adjacent to the restrooms and is inaccessible to clients. Clients & Staff Files . Will be stored electronically. Medications . Facility will not handle medication. First-Aid Kit & Book. Facility has a first aid kit mounted on the wall of the gym, and two first aid backpacks mounted throughout the facility equipped with the first aid
