Facility Evaluation Report

Licensing Program Analyst (LPA) Jessica Cho made an unannounced visit for the purpose of interviewing Client #1 (C1) in connection to a complaint investigation at another licensed facility for Complaint Control Number: 22-AS-20230320140108.

During today's visit, LPA interviewed C1 and Staff #1 (S1) and requested copies of pertinent documentation removed from client file. LPA observed four clients and four staff on duty. House Manager Alpizar was advised to submit copies of the documents via email by 10am today.

An exit interview was conducted with House Manager Delleneiry Alpizar, and a copy of this report was provided at exit via email.
