On February 4, 2026, at 12:45 PM, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced case management visit for complaint #22-AS-20210621141229 for a Closed Facility Mercedes Diaz Homes Inc- Ember (306005484) at the current re-licensed facility. LPA Kim met with Administrator (ADMIN) Maricela Gonzalez and explained the purpose of the visit.
LPA Kim conducted a tour of the facility with ADMIN Gonzalez. LPA observed 4 clients and 6 staff on site at the time of the visit.
No deficiencies were cited during the visit.
An exit interview was conducted and a copy of this report was provided to Administrator Maricela Gonzalez
