Facility Evaluation Report

This unannounced inspection is being conducted by Licensing Program Analyst (LPA) Sean Haddad for the purpose of conducting additional interviews for Complaint Control No. 22-AS-20220907180032. LPA met with Administrator (AD) Hyo Sook Kim and explained the reason for today’s inspection.

During the inspection, LPA interviewed AD and requested and reviewed copies of the resident roster and staff roster. AD identified 8 residents who regularly attend the church service activity in the common room. LPA interviewed 2 of these residents.

Facility representative was advised that at this time further investigation may be required. An exit interview was conducted and a copy of this report was discussed with and provided to facility representative.
