Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced case management visit to deliver an amended report. LPA was greeted and granted entry into the facility and explained the reason for the visit.
During the visit, LPA toured the facility and observed the following: Facility appears clean and sanitary. LPA observed satisfactory food supply and electricity and water is operational today. Residents are relaxing and appear clean.
LPA delivered an amended report for complaint #22-AS-20260126160621.
Exit interview conducted and a copy of this report was left at the facility.
