Facility Evaluation Report
Licensing Program Analyst (LPA) Celine Rodriguez made an unannounced visit to the facility to conduct the required annual inspection. LPA Rodriguez explained the purpose of today's visit and was greeted by staff on duty. For this visit, LPA Rodriguez met with Program Director (PD) Tala Fazeli, Assistant Program Director (APD) Jordan Herman, Operations Manager (OM) Jillian Rivero.
The facility is an Adult Day Program (ADP) licensed for 140 ambulatory clients for the ages of 18-59. The facility operates from Monday through Friday, 9:00AM-3:00PM. The facility is a two story building. The first level of the facility includes a reception area, client activity areas, and four bathrooms. The second story of the facility includes staff offices and additional activity areas. LPA Rodriguez conducted a tour of the facility and observed that there were clients engaging in activities with staff. LPA Rodriguez observed that toxins are stored in a locked storage cabinet. LPA observed a First Aid Kit to be stored in a cabinet near the reception area and it had all the required components. LPA Rodriguez inspected the four bathrooms and observed them to be clear of hazards. Faucets and toilets were operational. Hot water temperature measured between 110.7 and 111.4 degrees Fahrenheit. It was confirmed that facility does not handle medications or client's personal and incidental expense monies (P&I).
There are three fire extinguishers located in multiple areas of the facility and dated for 2025. LPA Rodriguez observed the Fire Service Marshall conducted their annual inspection on September 19, 2024, and is scheduled for another inspection in 2025 with a date pending.
