Facility Evaluation Report
On August 5, 2026, at 8:00 AM Licensing Program Analyst (LPA) Edward Kim conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Kim met with Assistant Program Director Jordan Herman and explained the purpose of the visit.
LPA Kim toured inside of the facility. Facility is a two-story commercial building. There are no food supplies or sharps in the facility. There are four bathrooms located on the first floor with all having working wash basin, soap, paper towels, and toilet paper. Hot water measured between 108.5 degrees F to 119.7 degrees F. On the first-floor LPA observed an activity area, a glass room area that is used for classroom and training, and two open warehouse areas. The back entrance is used for clients to check-in and the main entrance is used for staff to enter. The second floor has two enclosed office areas for staff and there is also an activity area dedicated to the clients with a large screen television, table tennis, foosball, and abundant seating area. LPA observed that the facility fire clearance is maintained in conformity with the State Fire Marshall regulatory standards. Smoke detectors and sprinkler systems are serviced annually by an outside vendor with an inspection date on September 11, 2025. There are three Fire extinguishers that are charged and all serviced on December 22, 2025. Certificate of Liability Insurance is effective from June 12, 2026, to June 12, 2027.
There are no bodies of water or obstruction at this location. Disinfectants cleaning solutions and poisons are inaccessible to clients and stored locked in a cabinet in the warehouse area. The premises and furnishings are in good repair. There are no food preparation areas at this site. Clients provide their own lunches and/or go out for lunch. Facility does not handle medication for clients and therefore does not have a location for medication storage.
