Facility Evaluation Report
Licensing Program Analyst (LPA) William Vanegas made an unannounced visit to conduct the required annual inspection. LPA met with Operations Manager (OM) Tiare Eda and President (PR) Moroe Fazeli. LPA explained the reason for the visit. LPA toured the facility and observed the following.
Facility is an adult day program with a capacity of 70 ambulatory clients. Facility is a 2 story building with multiple activity rooms, offices, conference rooms, and 4 bathrooms. During the visit 14 clients were present at the facility. During the visit all staff present at the facility were background cleared and associated to the facility. LPA observed that all fire extinguishers in the building are fully charged and up to date. LPA observed clients engaged in various activities such as arts and crafts, puzzles, and reading. LPA observed all bathrooms are clean and operational, hot water measured 120.8 degrees Fahrenheit. LPAs observed the kitchenette is clean and organized.
The refrigerator is clean and operational. There is a microwave oven that is operational and a sink. There is no stove in the kitchenette. LPA observed the back exit door is operational. The last fire drill was conducted on May 09, 2025. LPA did not observe any obstacles or hazards inside the facility.
LPA reviewed five staff files. All five staff members had CPR/first aid training and had all the required training. No discrepancies observed in the staff files. LPA reviewed seven client files. No discrepancies observed. Based on observations made during today's visit no deficiencies will be issued per tittle 22 division 6 of the California Code of Regulations. An exit interview was conducted with PR and OM and a copy of this report was left at the facility, and a copy will be mailed to the facility. .
